respite Residential Services
Each respite home is centered around the uniqueness of the individuals that reside there. Our group home is operated with integrity. Each individual has their own bedroom decorated with the resident in mind. Each home is warm and inviting, and, a safe place you can truly call home.
It is the goal of every staff and supported individual to give people the opportunity to live as independently as possible. Our residential services are a learning opportunity for everyone involved. We are evolving from the “caretaker” mentality to the “mentor” mentality.
Our #1 priority is the long-term success of the adolescents that we serve. We want to see them succeed in life and pursue their dreams and goals. Our residential facilities are staffed with trained service providers who care about the youth in our homes. We ensure the homes are monitored and safe to provide a stable, healthy environment for kids who would otherwise have nowhere else to turn to.
All services are provided with a focus on individuals enhancing their capacities to become as self-reliant as possible. Specific services offered through Residential Support include:
Personal care
In-home skills training
Health and medications management
Housekeeping/laundry
Community involvement
Meal planning/preparation
Financial management
On call emergency support via cellular phones and/or pagers
Transportation, when indicated
These services are provided 365 days a year with specific support schedules developed based on each individual's assessed needs. All services provided by our residential staff focus on our mission "working toward a community where each belong." Staff work with individuals to discover their interests and desires regarding community activities and then assist in facilitating engagement in these activities.
Admissions Process
Step 1 – Gather All Documentation Needed
The following documentation is needed in order to complete an application :
Current Psychiatric Evaluation and/or Assessment
Person Centered Plan from current program
Complete Residential Intake Application
List of Current Medications, a 30 day supply in hand
along with prescriptions from their doctor with at least
a 60 day supply of all required medications
Step 2 – Submit Referral and Documentation

The Clinical Team will review the information within 24 hours. An admissions Coordinator will contact you to discuss the referral and to make arrangements for submitting a Service Authorization Request.
Supporting Documentation (needed if accepted):
Birth Certificate and Social Security Card
Immunization Record
Court order of guardianship (if dependent)
Updated PCP reflecting Residential level III Service with a physician order (Physician Signature)
Comprehensive Clinical Assessment with a recommendation for Residential level III Service
Discharge Plan ( located under the Forms tab @ top of page Click Admissions then Forms)
Calocus ( located under the Forms tab @ top of page Click Admissions then Forms)
School information, including IEP/504 ( Please include Name Address & Phone Number of current school or last school attended with last date of attendance
Step 3 – Admission
Once Prior Authorization has been approved The legal guardian will need to sign admission paperwork prior to or at the time of admission. At the time of admission the individual will need to bring a 30 day supply and prescription for a 60 day supply for all medications to be filled at the local pharmacy and two weeks of weather appropriate clothing.
Clothing items accepted:
Ten Tops - Ten bottoms - Ten T - shirts - 3 Sets of pajamas - One Robe - 1 pair bedroom slippers - 1 pair of shower shoes - Twenty pairs of undergarments including (socks)
Five books - One book bag - One Jacket
